The Lawrence General Hospital Medical Records Department keeps patient information for both inpatient and outpatient services.  Our office is open from 8 a.m. to 4 p.m. Monday through Friday and can be reached at (978) 683-4000, x2047.
 
How we protect the privacy of your Protected Health Information (PHI)
Lawrence General Hospital is committed to respecting and protecting the rights of our patients and families. The privacy of your health information is very important to us, and we make every effort to ensure that it is kept confidential.

How to view your protected information
The Lawrence General Hospital Patient Portal (patientportal.lawrencegeneral.org) is a secure, easy-to-use way for patients to access their health information online.
 
How to obtain or release a copy of your health information
You must submit a formal request before we can release your health information. Please note that different forms are used if you are requesting your own record or are requesting that we release the health information to another provider. 
To submit written permission please complete, sign, and mail/fax/email us the following forms:

Please complete this form to request LGH to disclose your health information to another provider or third party
Please complete this form to request an amendment to your health information

All requests will be processed within 10 business days of receipt.  
 
Charges for copies
There is no charge for copies requested by health care providers or those needed for consultation or continuing care. There is a charge for copies that are requested for personal reasons. Attorneys and Insurers requesting records will be billed for the copies according to the number of pages in the record being requested. Please contact the Medical Records Department regarding the annual update to the copy fee charge.
 
Obtaining a Copy of Your Baby’s Birth Certificate
Massachusetts hospitals do not retain legal copies of birth certificates. Requests for legal copies should be directed to:
