Our caregivers have extensive experience caring for families, from pediatrics to geriatrics, and everyone in between – health care you and your family can trust.
Open Access Scheduling™, which means appointments are made on the day and time that works best for the patient, with same day and next day appointments are available as well. We also offer continuity of care, by coordinating your care at the hospital, in the event that you are ever admitted.

Our Providers
Lawrence, MA
Dr. Besher Jazmati joined the cardiology team of Community Medical Associates in December 2024 and works out of our Lawrence office. He attended medical school at Aleppo University in Syria and furthered his training in internal medicine at The Miriam Hospital in Providence, Rhode Island. Dr. Jazmati also studied nuclear medicine at the Harvard Joint Program of Nuclear Medicine, Brigham and Women’s Hospital and Children’s Hospital. His cardiology background includes stints at the Lown Cardiovascular Center, Brigham and Women’s Hospital, and Harvard Medical School. He has also worked in electrophysiology at Lahey Clinic in Burlington. Dr. Jazmati is certified by the American Board of Nuclear Medicine, American Board of Internal Medicine and the American Board of Cardiovascular Disease. His clinical interests include coronary artery disease, cardiac arrhythmias, heart failure and preventative cardiology.
Shilpa Monga, MD
Peter Ofman, MD, MSc, FACC, joined the Lawrence cardiology office of Community Medical Associates in December 2024, and focuses on general cardiology and cardiac electrophysiology. Board certified in cardiovascular medicine, clinical cardiac electrophysiology, internal medicine, echocardiography and nuclear cardiology, Dr. Ofman has a special interest in cardiac arrhythmias and preventative cardiology. He completed fellowship training at St. Vincent Hospital, the Boston VA, Brigham and Women’s Hospital and Harvard Medical School, in addition to Baystate Medical Center and Tufts Medical School. Dr. Ofman’s residency was at Brown University. He holds a medical degree from the Medical College of Virginia. Dr. Ofman speaks Russian and Hebrew.
Fernando Romero, MD
Suhaiti Villa, NP, is an American Association of Nurse Practitioners (AANP) board-certified family nurse practitioner who graduated from Fitchburg State University with a bachelor's degree in nursing in 2011 and started her nursing career at a family health center. She received her master's degree in nursing from Regis College in 2021, where she specialized in the family nurse practitioner program. Her interests include primary care throughout the patients' lifespan, women's health, and preventative care. Suhaiti is fluent in Spanish and English.

Torkom Garabedian, MD, is a primary care physician practicing at CMA Primary Care in North Andover. Board-certified in internal medicine, his clinical interests include preventive cardiovascular health and chronic management of cardiovascular conditions. Dr. Garabedian speaks English, Armenian, French, and some Arabic.
Garo Garabedian, MD, is a primary care physician, board-certified in internal medicine and obesity medicine. His clinical interests include weight management, hyperinsulinemia and insulin resistance, diabetes mellitus type 2 and cardiovascular prevention. Dr. Garabedian speaks English, Armenian, and French.
Stella Athanasopoulos, MSN, RN, FNP-BC is a family nurse practitioner. She received her master’s degree in nursing from the Boston College Connell School of Nursing and spent three years serving the Dorchester community at a local health center. She’s passionate about disease prevention, chronic disease management and the treatment of illness.
North Andover, MA (Cardiology)
Sunit Mukherjee, MD, is board-certified in cardiology and interventional cardiology. He earned his medical degree from the Boston University School of Medicine and has been in practice for more than 20 years. He speaks English and Bengali.
Cheryl Puello, MSN, APRN, FNP-C is a family nurse practitioner who began her nursing career in 2006. She initially specialized in emergency medicine, ICU, CCU, neuro ICU, cardiac catheterization laboratory, and interventional radiology. Cheryl later earned her master's degree from Rivier University in Nashua, N.H. Specializing in both inpatient and outpatient general cardiovascular medicine, Cheryl is dedicated to providing comprehensive care to patients with a wide range of heart-related conditions. Fluent in English and Spanish, she is committed to providing compassionate, high-quality care to a diverse patient population.
Andover, MA (General Surgery)
Dr. Syed Abbas brings with him a wealth of experience and a distinguished career in the medical field. Dr. Abbas completed his residency at SUNY - Stony Brook and later completed specialized training at the University of Florida in Jacksonville, focusing on advanced minimally invasive surgery and surgical oncology. He also holds a master’s in business administration from Auburn University. Dr. Abbas’ commitment to continuous learning and professional development is exemplified by his active memberships in esteemed organizations such as the American College of Surgeons, the Society of American Gastrointestinal and Endoscopic Surgeons, and the American College of Healthcare Executives. Here at Lawrence General, Dr. Abbas assists with surgical oncology, colorectal surgery, trauma surgery and laparoscopic surgery.
Dr. Kaitlan Ahrens graduated from the University of Toledo College of Medicine in 2016 and went on to complete a surgical residency at UMass Memorial Medical Center in 2023. She completed a post-doctoral research fellowship at Massachusetts General Hospital 2020 investigating the mechanisms of tolerance in heart transplantation.

Dr. Nancy Cho Landay completed her residency in General Surgery at Beth Israel Hospital (now Beth Israel Deaconess Medical Center) in Boston. She then completed a one-year position as Surgical Coordinator for Beth Israel’s Department of Surgery as a staff surgeon and Instructor of Surgery at Harvard Medical School. A longtime Andover resident, Dr. Landay is a Yoga Alliance-registered yoga teacher (RYT 200) and enjoys singing with local choruses.
Earl Gonzales, MD, has over 15 years of experience in general surgery. He is one of the rare breed of surgeons who are well-trained in both open cases as well as minimally invasive surgery. He has special interests in laparoscopic colorectal, complex hernia surgery, acute care and trauma.
Gabrielle Guetta, PA, graduated from Colgate University with a bachelor’s degree in molecular biology and education. After college, she spent time living and working in Manhattan. She then moved to Boston to attend Boston University School of Medicine’s Physician Assistant Program and worked as part of Brigham & Women’s Hospital’s surgical access team. Gabrielle joined Andover Surgical Associates in January 2023 in an inpatient-based role, where she cares for patients during their stay at Lawrence General Hospital. She sees consults in the Emergency Department, manages patients on the floors and assists surgeons in the operating room.
Dr. David Ma is a skilled general surgeon specializing in colon and rectal surgery. He trained in general surgery at St. Elizabeth’s Medical Center and completed his colon and rectal surgery fellowship at Rutgers University – Robert Wood Johnson. A member of the American Society of Colon and Rectal Surgeons and the Massachusetts Medical Society, Dr. Ma actively contributes to the medical community and health care policy. His patient-centered approach ensures that individuals receive personalized care tailored to their unique needs.

Beth Allison, MS, RD, LDN, holds a master’s in nutrition from Framingham State University. She has over 20 years’ experience in weight management counseling and a passion for healthy cooking. As a dietitian, she provides supportive, compassionate care and motivates patients to find success through their weight loss journey.
Avery Burns, MPH, RD, LDN, received her master’s degree in dietetics from the University of Massachusetts: Lowell and works alongside Beth Allison at the Weight Management & Bariatric Center at Lawrence General Hospital. Her passion is helping patients reach their nutrition goals and improving their quality of life. When she’s not working, she enjoys exercising and trying healthier alternative recipes.
Dr. Michelle Nessen is a board certified general surgeon with specialty training in Advanced GI/MIS and Bariatric Surgery. Her clinical interests include bariatric and foregut surgery, hernia repair, gallbladder disease and surgical education. Dr. Nessen graduated from Amherst College in Amherst, MA, with a degree in neuroscience before receiving her medical degree at the University of Rochester School of Medicine and Dentistry in 2018. She completed her general surgery internship and residency at Montefiore Medical Center in Bronx, New York, before going to complete fellowship at Tulane University in New Orleans. She is trained in both laparoscopic and robotic techniques, and advanced bariatric procedures including the duodenal switch. Throughout her training, she won multiple clinical and teaching awards and has presented her work at various national meetings including the American College of Surgeons, SAGES and ASMBS.
Ketna Patel, PA, received her master's degree in physician assistant studies from the Massachusetts College of Pharmacy and Health Sciences. She has over 10 years of experience caring for bariatric patients. With her role as a physician assistant and program coordinator at the Weight Management & Bariatric Center at Lawrence General Hospital, she strives to provide compassionate care and education to guide patients through their weight loss journey.
Caitlin Polistena, MD, trained at Montefiore Medical Center in New York and went on to complete a minimally invasive and bariatric surgery fellowship at Cleveland Clinic Florida thereafter. Dr. Polistena is a member of the American College of Surgeons and the American Society for Metabolic and Bariatric Surgery.
Roy Shen, MD, FACS, FASMBS, trained at Jefferson Medical College in Philadelphia and New York University Hospital before coming to Massachusetts in 2003. He has extensive experience in laparoscopic bariatric procedures, providing training to other surgeons. Dr. Shen is a fellow of both the American College of Surgeons and the American Society for Metabolic and Bariatric Surgery.

Jonathan Gordon, MD, is a board-certified general and vascular surgeon with more than 25 years of experience in surgery, wound care, and vascular medicine. He serves as chief of surgery at Holy Family Hospital and medical director of Holy Family’s Wound Care and Hyperbaric Center. Dr. Gordon earned his medical degree from New York Medical College and completed his surgical residency at Boston University, where he served as administrative chief resident. He then pursued fellowship training in vascular surgery at the Boston Veterans Administration Medical Center. Dr. Gordon holds board certifications in general surgery, undersea and hyperbaric medicine, and wound medicine and surgery. He is a fellow of both the American College of Surgeons and the American College of Clinical Wound Specialists.
Liam Hurley, MD, is a board-certified urologist with over 30 years of experience delivering care to patients across the Merrimack Valley and southern New Hampshire. A Harvard graduate and Boston University-trained physician, Dr. Hurley has served as chief resident at Lahey Clinic and held faculty appointments at Boston University, Tufts University, and Lahey Clinic Medical Center. His clinical interests span the full spectrum of urologic care, including surgical innovation and prostate cancer treatment and long-term outcomes in brachytherapy. Dr. Hurley is a fellow of the American College of Surgeons and has held leadership roles in multiple professional societies, including president of the Boston Urological Society.
Paul Karam, MD, specializes in bariatric, foregut, and general surgery with roughly a decade of rigorous post-graduate training. He is a certified fellow with the American Society for Metabolic and Bariatric Surgery and is currently obtaining a fellowship certification in foregut surgery. Dr. Karam received his Doctor of Medicine degree at Holy Spirit University of Kaslik in Lebanon and completed his bariatrics and foregut fellowship at Saint Luke’s University Health Network. He also completed a general surgery residency at the Cleveland Clinic Foundation. Dr. Karam speaks English, French and Arabic.
Hossein Keyhan, MD, specializes in urology and urologic surgery. He completed two clinical fellowships – one in Endourology & Robotic Urology at St. Elizabeth Medical Center, Boston University School of Medicine, and another in ASTS Accredited Kidney Transplant Surgery at University Hospital, Cleveland Medical Center / Case Western Reserve University School of Medicine in Cleveland, Ohio. His residency was in urology at the Tehran School of Medical Sciences in Iran, the school from which he also received his Doctor of Medicine degree. Dr. Keyhan speaks English and Farsi.
Jeffrey Lord, MD, FACS, FASMBS, is a board-certified general surgeon and fellowship-trained expert in minimally invasive and bariatric surgery. A decorated veteran of the United States Navy, Dr. Lord has held numerous leadership roles in both military and hospital settings, most recently serving as director of bariatrics and MIS surgery at the Sacred Heart Institute for Surgical Weight Loss, part of Ascension Sacred Heart Pensacola in Florida. Dr. Lord also serves as president of Bariatric Consulting Group and recently worked as a physician with North Florida Surgeons. He is widely recognized for his expertise in laparoscopic surgery and has published extensively on topics ranging from bariatric procedures to surgical education and outcomes. Dr. Lord received his Doctor of Medicine degree from Wayne State University School of Medicine and completed his fellowship at the University of Pittsburgh Medical Center.
Catherine Tucker, MD, is a board-certified breast surgical oncologist with more than 25 years of experience in breast surgery, surgical oncology, and breast health. Dr. Tucker earned her medical degree from Tufts University School of Medicine, where she was elected to the Alpha Omega Alpha Honor Medical Society and received multiple honors, including the Martin J. Loeb Memorial Prize in Surgery and the Janet M. Glasgow Memorial Achievement Award. She completed her residency training at both Tufts-New England Medical Center and NYU Medical Center, followed by a fellowship in breast diseases at the Dana-Farber Cancer Institute/Faulkner Breast Center. She has served in leadership roles throughout her career, including medical director of the Breast Care Center at Winchester Hospital and director of the breast health program at North Shore Medical Center. Dr. Tucker is a fellow of the American College of Surgeons and a member of the American Society of Breast Surgeons, with certifications in both breast ultrasound and stereotactic biopsy. In addition to her clinical work, Dr. Tucker has contributed to breast cancer research, published in peer-reviewed journals, and presented extensively to both professional and community audiences on topics related to breast health, cancer detection, and surgical advances.

Shruti Biyani MD
Judi Carboni, NP, is a certified family nurse practitioner with over 20 years of experience providing comprehensive obstetric and gynecologic care. She earned her Master of Science in nursing from the Massachusetts General Hospital Institute of Health Professions and holds a bachelor’s degree from the University of New Hampshire. Carboni’s expertise includes annual exams, prenatal and postpartum care, STI screening, contraception counseling, and menopause management.
Brooke Guthrie, NP, is an experienced nurse practitioner specializing in women’s health. She has been caring for patients across the Merrimack Valley for over 20 years, with a focus on obstetrics, gynecology, and preventive health.Brooke earned her Master of Science in Nursing from the Massachusetts General Hospital Institute of Health Professions with a dual focus in adult and women’s health. She currently provides comprehensive care to women of all ages at Community Medical Associates in Salem, New Hampshire, including wellness exams, pregnancy care, contraception counseling, and gynecologic evaluations. Her approach is compassionate, collaborative, and patient-centered, with an emphasis on long-term wellness and personalized support.
Lan Tran, MD, is a board-certified obstetrician-gynecologist and a fellow of the American College of Obstetricians and Gynecologists. She earned her medical degree from the University of Rochester School of Medicine and Dentistry and completed her residency in Obstetrics and Gynecology at William Beaumont Hospital in Royal Oak, Michigan, where she received the Resident Achievement Award from the Society of Laparoendoscopic Surgeons for excellence in surgical skills. Dr. Tran has been practicing since 2012, with past clinical experience at Carney Hospital, St. Elizabeth Medical Center and St. Joseph Hospital in Nashua, New Hampshire. She received a degree in mathematics and pre-med from the College of the Holy Cross, where she graduated cum laude and was inducted into multiple national honor societies. Dr. Tran has participated in clinical research on nephrolithiasis in pregnancy and previously held research and clinical roles at Massachusetts General Hospital, Mass Eye and Ear Infirmary, and St. Elizabeth Medical Center.
