How to Request Films/CD

Films/CDs may be picked up at the Main Radiology Department (second floor) seven days
a week.

To Expedite the Process – Please Call or Fax Ahead:

• By phone: 978-946-8103
• By fax: 978-946-8067

Please Provide the Following:

• Your full name
• Your date of birth
• Date exam was done
• Type of exam you are requesting
• When you would like to pick them up
• Name of the doctor you are bringing them to

Hours for Pick-Up:

• Monday - Friday: 7:00 a.m. - 8:00 p.m.
• Saturdays & Sunday: 7:30 a.m. - 4:00 p.m.

What to Bring:

• Photo ID
• If you are having someone pick up the films/CD for you, please send a note stating that you give permission to have your records released to them.

You do not need to return the films/CD to the hospital. You may retain for future doctor's appointments.
